If you are canceling an appointment with less than 24 hours notice or are running late, please be advised you will be billed for that appointment.

If you are calling or texting to leave a message just prior to your appointment, please be aware that I may be in session and unable to respond to your call at that time.
If you are running late, please call or text me and be aware that I may not be able to offer you the entire length of your session if you are arriving late. I will accommodate you to the best of my ability.
Please remember that while circumstances (emergency or otherwise) do arise that prevent patients from keeping a scheduled appointment, I cannot absorb the financial cost of all of my patients’ missed appointments. Since scheduling of an appointment involves the reservation of time specifically for you, a minimum of 24 hours notice is required for re-scheduling or canceling an appointment. Appointments not cancelled by voice mail, text, or email with 24 hours notice will be billed in full regardless of the circumstances.
Therapy sessions are billed at $175.00 for 45 minutes and payment by check or cash is required at the time of the session. I do not accept credit cards or Pay Pal.
Consultations, telephone conversations in excess of ten minutes, consultation time with other professionals who have knowledge of the patient, site visits, review of records submitted, report writing, and travel time, etc. will be charged at the same rate of $175.00 per each 45 minutes.
Patients who carry health insurance should remember that professional services rendered are charged to the patient at the time of the service and not to the insurance companies. I will provide you with a copy of your receipt for each session at the time of the session. The receipt can then be submitted by you to your insurance company for reimbursement if you so choose. Because I am not a provider for any insurance companies, be sure to verify your coverage stating that you are using an “out of network” provider.
My “out of network” provider number or NPI # for your insurance claims is: 1194972752. Many insurance companies do partially reimburse policy holders for “out of network” charges related to mental health. Check with your insurance company as to the amount they will reimburse you for when you use an “out of network” psychologist. Remember to factor in that there is no co-pay on your part when you use an “out of network” provider. The insurance claims CPT code number used by insurance companies for processing claims for individual psychotherapy is 90806. I will provide you with a receipt for your psychotherapy billed under this code. Again, it is your responsibility to verify the specifics of your coverage and to seek reimbursement from your insurance company. I am not a party to any financial settlement between patients (policy holders) and insurance companies.
Formal Evaluations (Testing) Sessions are billed at $200.00 per hour.
The process of a thorough psychological evaluation is formal and lengthy, generally involving about 20 hours of the psychologist’s time. For formal evaluations, patients are expected to pay by check the full evaluation fee of $200.00 per billed hour.
A $1500.00 initial payment is due by mail with paper work filled out by patient and/or parents one week prior to the first testing session. The balance is due at the time of the follow-up consultation. Complete evaluations generally run between $2750.00 and $3000.00 for the total hours involved which include testing time, scoring time, analysis time, follow-up consultation time, and written report time. Telephone conversations and consultation time with other professionals who have knowledge of the patient, site visits, review of records, report writing, and travel time, etc. will be charged at the same rate of $200.00 per hour.
